Custom forms are invaluable tools for collecting specific information efficiently. Whether you're gathering data from employees or customers, creating a custom form can streamline the process. This guide will walk you through the steps to create a custom form or click here to view a video.
Log in to your portal or platform. Navigate to the Forms section, typically located on the left-hand side of your portal.
Click on Creating Form.
Add a title for your form. This title will help identify the form's purpose.
Select the specific form fields you need for your form. You can include various field types such as:

- Short text field
- Long text field
- Multiple-choice
- Date picker
- Number field
- Heading
- File and image upload
- Time
- Signature
- Table
- Email address
For each field, you can click Edit to customise the field title, designate whether it's mandatory, and configure other settings as needed.
If you include an email address field, you can toggle on Send email copy if desired.
You can also enable the Link to Job option, which adds a field to your form where users can select the job to link the submission to. You can make this field mandatory during editing.
Customise form access by specifying which departments should have access then select by role.
After configuring your form to your satisfaction, click Save as Draft. This action will save your form's progress.
To make your form available for use, go to the Forms page.
Locate your form and select it by clicking in the box next to it.
Click Actions and choose Publish.
To access submissions across the entire company, navigate to the Submissions tab. By selecting the three dots icon, you have the option to either export the form as a PDF or archive it. To export all submissions at once, click on Export All Submissions. Additionally, you can apply filters to narrow down submissions based on staff, job number, submission date, or status.
If a form has been configured to be associated with a specific job, you can access its submissions by entering the corresponding job and selecting the Forms Submissions tab. Within this section, you also have the option to export individual submissions as PDFs or export all of them at once.
